Project Description
The programme entails three major hospital projects in Cape Town: the new Klipfontein and Belhar regional hospitals and replacing of the existing Tygerberg Hospital with a new Tygerberg Central Hospital.

The Klipfontein and Belhar regional hospitals are planned as new facilities that will strengthen the province's regional hospital network, bring services closer to growing communities and ease pressure on existing hospitals.

The Klipfontein Regional Hospital, planned for Manenberg, will serve the Cape Town hubs of Athlone, Gugulethu, Hanover Park, Manenberg, Mitchell's Plain and Philippi and, based on 2024 planning, is expected to provide care for between 3 500 and 4 000 people a day.

Klipfontein will be developed in phases. Phase 1 is planned to commission 222 beds and build space for a further 196 beds. Phase 2 will commission the beds, while Phase 3 will add further wards, taking the hospital to a maximum capacity of 640 beds.

The Belhar Regional Hospital is planned as a 642-bed facility serving Cape Town's Tygerberg substructure, the Khayelitsha and Eastern substructures, and the Stellenbosch subdistrict. An estimated 3 500 to 4 000 people a day are expected to use the hospital. Regional and district services provided at Tygerberg Hospital are intended to relocate to Belhar once the new facility is operational.

The new Tygerberg Central Hospital will be an 893-bed tertiary and quaternary facility on the western side of the existing Tygerberg Hospital campus. It will be delivered through a public–private partnership (PPP), with the private partner responsible for designing, financing, building and maintaining the hospital over the contract period. Clinical healthcare services, health technology and furniture, as well as the existing X-Block, which includes 47 oncology beds, are excluded from the PPP.

Capital Expenditure
The new Tygerberg Central Hospital is estimated at R10-billion, including equipment. The Western Cape Government plans to invest R18.5-billion in metro healthcare infrastructure over the next decade, with the Tygerberg, Klipfontein and Belhar projects forming part of this programme. Current individual capital estimates for Klipfontein and Belhar have not been confirmed.

Planned Start/End Date
Klipfontein and Belhar remain targeted for completion in 2033. 

The request for proposals for Tygerberg is planned for July 2027, with selection of a preferred bidder targeted for July 2028 and financial close for April 2029. Construction is expected to take about 4.5 years, with commissioning targeted for the 2033/34 financial year.

Latest Developments
The Western Cape Government's 2026/27 health budget confirms that the design and planning for the Tygerberg redevelopment and the new Metro East (Belhar) and Metro West (Klipfontein) regional hospitals are under way. The Health Facilities Management budget increased to R1.72-billion, largely owing to the funding of the Budget Facility for Infrastructure's supporting major healthcare infrastructure projects.

For Tygerberg, enablement work continued during 2026 to meet the procurement requirements for the PPP. The project received National Treasury Approval I in November 2022 and remains in the procurement phase.

A professional service provider team has also been appointed for Belhar, although its members have not been disclosed. The Tygerberg PPP private partner has not yet been appointed.
